application, have you received treatment (including medication ordered by a Physician)
or been diagnosed with a medical condition other than a Minor Ailment?
For Persons 60-69 years:
If you answer "yes" to question
1, you are eligible for Option A.
If you answer "no" to question
1, but "yes" to question 2, you are eligible for Option B.
If you answer "no" to both questions
1 and 2, you are eligible for Option C.
For Persons 70 - 75 years:
If you answer "yes" to question
1, you are not eligible to purchase this insurance.
